【 要旨 】 欧米の生体肝移植の事情を調査した．ヨーロッパでは少しずつ成人生体肝移植が増えつつあるが，脳死肝移植に比べてドナーの合併症やグラフト喪失率が高い．一方，アメリカでは2001年をピークに生体肝移植が減少傾向にある．この要因としてMELDスコアの参入，それに伴う肝細胞癌の脳死肝移植の増加とドナーの死亡などが考えられている．

[ Summary ] The author researched the trend of living donor liver transplantation in Europe and the USA. In Europe, the numbers of living donors has gradually increased, especially in adult candidates. However, morbidity and mortality rates are as high as 21 % and 0.5 %.
On the contrary, in the USA, the dramatic increase in the number of living donor liver transplants has now declined because of the implementation of the MELD scoring system, HCC and decreased mortality for liver donors.
