【 要旨 】 内視鏡的粘膜切除術 (EMR) の適応決定,表在食道癌へのEMR+α治療,進行癌に対する手術や化学放射線療法 (CRT) の選択など,治療方針の決定には正確な深達度とリンパ節転移診断が不可欠である.表在癌の深達度診断には細径超音波プローブを用いるが,sm以深癌には超音波内視鏡 (EUS) 専用機が適している.誤診例は少なく,他臓器浸潤診断もCTより精度が高い.系統的な頸胸腹3領域リンパ節の検索は周囲臓器や血管との位置関係から判断するため,EUS専用機を用いる必要がある.微小転移の診断には限界があるが,CTや超音波検査の2 - 3倍の転移リンパ節を診断できる.治療方針の決定に直結する場合にはEUS下穿刺生検 (EUS-FNAB) を行うが,定期的かつ長期的な経過観察が重要である.

Title Diagnosis of Depth of Invasion and Lymph Node Metastasis of Esophageal Cancer Using Endoscopic Ultrasonography

[ Summary ] Accurate diagnosis of invasion depth and lymph node metastasis in the 3 fields, the neck, the mediastinum and the abdomen, is important in making decisions on endoscopic mucosal resection (EMR) or EMR + α applications for superficial cancer, and in selecting the type of operation or chemo-radiotherapy for advanced cases. High frequency miniature ultrasonographic (US) probes were used to evaluate the depth of superficial esophageal cancer. With the cancers deeper than sm, it is necessary to use conventional endoscopic ultrasonography (EUS) equipment, which provides better ultrasound penetration. In the diagnosis of invasion depth, EUS has a higher accuracy rate than CTs. This is also true in diagnosis of invasion of surrounding organs. Since systematic check ups for lymph node metastasis are evaluated with an eye on surrounding organs or vessels, it is necessary to use conventional EUS equipment. Although its diagnostic capability for minute metastasis is limited, 2 or 3 times more lymph node metastases have been found this way than that with CT or US examinations. However, since the diagnostic capability of EUS is also limited, we are performing EUS-guided fine needle aspiration biopsy (EUS-FNAB) if the images of lymph nodes are difficult to judge for a definite diagnosis, leading directly to decisions on therapeutic strategies.
