【 要旨 】 下部消化管の機能性疾患の診断や病態の把握のために行うおもな検査法について解説した．おもな検査法としては消化管通過時間測定，大腸運動の記録，大腸知覚検査，画像検査などがある．各検査は適応を判断して用いる必要がある．治療については過敏性腸症候群などの腸障害，および機能性便失禁などの直腸肛門障害に含まれる疾患について述べたが，各疾患とも確立した治療法に乏しい．機能異常の病態をよく理解し，機能異常を改善する薬物治療を行う．また，心理的因子の関連することも多く，向精神薬の投薬や心理療法を併用する．

[ Summary ] The aim of anorectal physiologic tests is the evaluation of pathophysiologic functions of the anorectum. Transit studies, colonic motility, anal manometry studies, rectal sensory tests, defecography, X-ray examination, etc. are performed in clinical settings. Anal manometry and defecograpy are important for the evaluation of anorectal disorders. This review presents information on the treatment of bowel disorders and anoretal disorders. Therapeutic options in treatment of these disorders must be based on pathophsiologic findings. Psychosocial factors are related many cases with these disorders. The usage of psychotropic drugs and psychotherapies may be necessary to alleviate the patients' symptoms.
