| 【 要旨 】 | いわゆる"慢性胃炎"は病理学的慢性胃炎，内視鏡的慢性胃炎，症候性慢性胃炎などが包括されたもので，今日，概念の混乱をきたしている．欧米ではこのなかから症候性慢性胃炎をNUD，FDなどの名称を付して分離独立させたが，わが国ではなおまとまっていない． 慢性胃炎の分類として1991年に公表されたSydney systemを，この問題の解決にいかに利用するかを，文献による現状の分析から検討した． その結果，現在でもSydney systemは，病理部門についてはかなり利用され，ある程度満足できるものとなっているが，内視鏡部門はなお未完成であり，現実に利用されることも少ないことが明らかになった．今後，この分類における個々の内視鏡所見がどのような意味をもつのか，組織像との関連，症状との関連も考慮して，さらに改変を加えていくことが求められる． |

| [ Summary ] | So-called "chronic gastritis" include histopathological gastritis, endoscopic gastritis and symptomatic gastritis, therefore, exact descriptions of this condition are now in a state of confusion. In western countries, symptomatic gastritis has recently been separated from other forms of this condition and has been termed non ulcer dyspepsia (NUD) or functional dyspepsia (FD). However, in Japan, it is still described using the former term. The "Sydney System" was published in 1991 as a new classification for chronic gastritis. This paper discusses how to use this system to eliminate this confused state of affairs. Up to this time, the pathological division of the Sydney system has frequently been used and has proven satisfactiory, but the endoscopic division of categories is still incomplete and is not useful. For better utilization it is necessary to clarify the meaning of each endoscopic finding in relation to the "Sydney System" and to consider the relationships between histological findings and symptoms. |
