We studied the distribution of intestinal metaplasia in the columnar-lined esophagus of 47 patients without Helicobacter pylori (H.pylori) infection with magnifying endoscopy, using methylene blue. In 2 patients with long segments of Barrett's esophagus, the longitudinal vessels followed the proximal margin of the gastric folds and intestinal metaplasia existed on the proximal side of the longitudinal vessels. Of the 45 patients with columnar-lined esophagus (less than 3 cm), 27 patients had intestinal metaplasia. There were 31 men, 23 (74%) who had intestinal metaplasia, but only 4 (29%) of the 14 women who had intestinal metaplasia. The columnar mucosa in the lower esophagus should be researched further because junctional cancer is reported to occurred in this area in patients without H.pylori infection. |
