| 【 要旨 】 | 大腸ポリープの臨床的取扱いの基本になるclean colonの概念について，第20回日本大腸検査学会 (平成14年11月，東京) におけるコンセンサス・ミーティングにおける討論結果を紹介した．そこではclean colonの定義として，2回連続のtotal colonoscopyでポリープが発見されなかった場合とすることでコンセンサスが得られた． しかし拡大内視鏡や色素散布で精密内視鏡検査を行うと，必ずポリープが発見されることから，clean colonはありえないとする意見もあったし，筆者も同感である．しかしclean colonの状態に到達しなくても，微小ポリープは無視してサーベイランス計画を実行することが現実的であるし，サーベイランスの間隔を延長するための努力も必要であることを強調した． |

| [ Summary ] | A result of the debate at the Consensus Meeting at 20th Congress of The Japan Society of Colon Examination about the concept of a "clean colon", which is the basis of clinical management of colorectal polyps, was presented. At the meeting, a consensus on the definition of a "clean colon" being cases where no polyps were found after two consecutive total colonoscopies. Some people are of the opinion that a "clean colon" is one where no polyps are found with magnifying colonoscopy or by the dye spraying method, with which the author agrees. However, using surveillance plans which disregard small polyps is reasonable and an important method to extend intervals of surveillance. |
