【 要旨 】 | 従来法では正確な切除ができず切除面積に制限があったが,endoscopic submucosal dissection (ESD) の開発により大きくかつ正確な一括切除が可能となった.われわれは76例の食道表在癌にESDを施行し,一括完全切除率は89.5% (68/76),局所再発率0% (0/76) であった.偶発症として穿孔はなく,縦隔気腫を5.3% (4/76) に認めたが保存的に軽快した.拡張術を要する狭窄を10.8% (8/74) に認め,1例1.3%にステントを一時的に挿入した. フックナイフによるESDでは大きくかつ正確な一括切除が可能で偶発症も許容範囲であり,食道表在癌に対する有力な治療法である. |

[ Summary ] | Conventional endoscopic mucosal resection (EMR) techniques make it possible to resect only small specimens. Therefor, piecemeal resection has been performed for large lesions. Now, we have developed a new endoluminal surgery method-endoscopic submucosal dissection (ESD). We cut the mucosa around the lesion with a hook knife, and dissect submucosal fibers, also. Then, a large lesion can be resected in one piece. We have resected seventy six cases superficial cancer with this new technique. The one piece resection rate was 89.50% (68/76). The major complication was mediastinal emphysema 5.30% (4/76), and there were no perforations. A more precise and larger one piece resection could be performed with ESD. Therefor, ESD with a hook knife is useful for treatment of superficial esophageal cancer. |
