【 要旨 】 胆道癌の診断における体外式超音波検査,とくにtissue harmonic imaging法(THI),パワードプラ法(PDI),造影エコー法(CEUS)の意義について解説した.THIではアーチファクトが低減されるばかりでなく分解能も向上するので,胆嚢底部病変や胆管病変の描出能に優れ,さらには癌の深達度診断などの精密診断にも有用である.PDIでは病変部や胆嚢壁の血流情報が精細にわかるので,癌と良性病変との鑑別診断や脈管浸潤などの進展度診断に応用されている.最近,急速に普及しつつあるCEUSでは病変部の血行動態とくに栄養血管や染影効果がさらによく把握できるので,腫瘍性病変の性状診断や癌の進展度診断などに期待が高まってきている.

[ Summary ] We reviewed the diagnostic utility of extracorporeal ultrasonography for biliary tract cancer. Tissue harmonic imaging (THI) has enable us to make some reductions in artifacts in addition to improvements in the spatial resolution of B-mode images. Therefore, polypoid and/or elevated lesions of the gallbladder may be clearly delineated. Additionally the wall-thickness of the gallbladder can be analyzed more precisely with THI combined with power Doppler imaging (PDI). PDI allowed us to archieve quantitative evaluation of blood flow in the cystic vessels; maximal velocity (Vmax), pulsatile index (PI), and resistance index (RI). From our results, these parameters seem to be available for differentiation of small polypoid lesions and wall-thickening changes.
Contrast-enhanced US (CEUS), using Levovist, is a new modality for evaluating hemodynamics. The main advantages of this imaging are thought to be acquisition of semi-real-time imaging of vasculatures and perfusion imaging of the tissue. CEUS is also efficient for the estimation of tumor extension in the gallbladder and bile duct cancer into adjacent organs: liver, pancreas, etc.
