| 【 要旨 】 | 肝外胆管を中心とした胆道癌の早期発見と進展度診断のための検査法の選択，また治療決定に有用な診断情報を迅速に得るための効率的診断体系を述べる． 早期診断には，胆管癌では胆道系酵素上昇例に，胆嚢癌では胆石症例に体外式腹部超音波検査を頻回に施行することが重要である． 胆道癌の治療の第一選択は外科的切除であるが，高度進行癌や合併症による切除不能例でもステント留置などによりQOLが改善している．迅速で正確な進展度診断にて手術適応の有無を早期に決定することが治療成績向上につながる． |

| [ Summary ] | This report describes useful examinations for early and accurate diagnosis of extrahepatic biliary tract carcinoma. Repeated ultrasonographies are important to detect early extrahepatic biliary tract carcinoma in cases of increasing serum transaminase or gall bladder stines. Surgical treatment is the first choice for care extra hepatic bile duct carcinoma, although in unresectable cases, stenting therapy contributes to improved quality of life. Rapid and accurate diagnosis extrahepatic bile duct carcinoma improves treament results. |
