【 要旨 】 原発性胆汁性肝硬変(PBC)発症の詳細な機序は依然として不明であるが,その家族集積性より遺伝的背景が一因すると考えられる.これまでにPBC発症の遺伝的背景としてHLAが広く検討され,HLA-DR8やそのsubtypeが疾患感受性遺伝子として認識されている.近年,いくつかのサイトカインや免疫因子の遺伝子多型とPBCの発症や病態の関連性が報告され,関連遺伝子の知見が集積されつつある.
PBCがpolygenic diseaseであることより,より多くの遺伝因子において検討が進み,疾患感受性因子や疾患進行性に関与する遺伝的因子の解明が望まれる.

Title Genetic Factors in Pathogenesis of Primary Biliary Cirrhosis

[ Summary ] The pathogenesis of primary biliary cirrhosis (PBC) remains uncertain. However, based on familial clustering of PBC, some genetic factors are considered to contribute to the onset and development of PBC. One of the factors, HLA-DR locus distribution, has been determined worldwide and the HLA-DR8 locus is believed to be an associating factor. Other factors, such as genetic polymorphisms of several cytokine and immunologic factor genes have been investigated. Some of these have a close association with PBC. However, based on these results, PBC is suspected of being a polygenic disease. In this review, we described the studies on HLA and genetic polymorphism of several cytokines such as TNF-alpha, IL-10, TGF-beta, and MBL in relation to the pathogenesis of PBC. Further genetic analysis is needed to define the specific host gene(s), having a close linkage with the onset and/or development of PBC.
