【 要旨 】 上部消化管疾患の診断におけるCTの役割は従来,他臓器への直接浸潤の有無や転移の有無の評価であったが,近年のCT,ワークステーションの進歩によって三次元的に小病変の描出が可能になった.また,軸位だけでなく多方向の断層像が再構成できるようになり,二次元診断もその幅と適応を大きく広げている.さらにdynamic enhancement studyを行い,多相の撮影をすることによって粘膜下腫瘍だけでなく,癌や潰瘍の質的診断にも寄与するところが大きく,ほかの検査ではわからない情報を得ることができる.スクリーニングとして日常臨床に定着するためには越えなければならないハードルは高いが,現時点でも術前の一検査法としての意義は大きい.

[ Summary ] The role of CT for diagnosis of upper gastrointestinal tract has been changing since the introduction of multidetector-row CT(MDCT). Three-dimensional(3D) CT of the upper gastrointestinal tract, so called virtual endoscopy, reconstructed with finer axial images, acquired with MDCT, have made it possible to demonstrate luminal images of gastric lesions very close to those produced by conventional endoscopy. Thus, with these images, we can assess the degree of tumor invasion into the gastric wall, as we have done with conventional endoscopy, and double contrast barium studies, by observing the margins of ulcers and converging folds. Moreover, multiplanar reconstructed images, using thinner axial images can assess tumor invasion more accurately than before and in some cases, it is possible to diagnose early gastric cancer two-dimensionally. A combination of three-dimensional and two-dimensional assessment of gastric cancer can lead to accurate diagnosis of tumor invasion. This has the potential for CT diagnosis of gastric cancer as one stop shopping in the diagnosis of gastric cancer.
