| 【 要旨 】 | EMRとERCPのクリニカルパス導入過程について，対比しながら考察した． EMRのクリニカルパスは，医師・看護婦・コメディカルがしっかりしたチームワークを形成，コミュニケーションをはかり作成したことから，実際の診療で広く活用されている．一方，ERCPのクリニカルパスは，スタッフ間の討議もないまま従来の検査手順書を改変し作成されたため，問題を残していた． クリニカルパスは，医師と看護婦ならびにコメディカルがチーム医療を確立し，より質の高い医療を実現すること，患者へのインフォームド・コンセントを深めることなどを主要な目的として導入するものであり，クリニカルパスを作成することだけを目標にしてはいけない． |

| [ Summary ] | This study dealt with the process for using clinical pathways, making a comparison between clinical pathways for the endoscopic mucosal resection (EMR) of early gastric cancer and that for endoscopic retrograde cholangio-pancreatography (ERCP). The concept of clinical pathways for EMR was introduced in numerous meetings with medical staff members of the gastroenterology unit over a 6 month period. On the other hand, the clinical pathway for ERCP was produced from a formely used manual for ERCP by a pathway committee, without staff meetings in the gastroenterology unit. The purpose of making clinical pathways is to establish a strong medical staff, to produce high quality medical service, and to receive "informed-consent" from patients. In the process of making clinical pathways, it is very important to establish a strong team of doctors, nurses and co-medical staff members, who can communicate with each other. Only clinical pathways produced through this process will hold the promise of being practical in real medical service. |
