【 要旨 】 大橋らの予後の良い膵癌としての「粘液産生膵癌」の報告以来，注目を集めてきた「粘液産生膵腫瘍」は1996年，WHOやAFIP分類で病理組織学的におもに「膵管内乳頭腫瘍」と「粘液性嚢胞腫瘍」とに分類され，その定義が報告された．一方，現在，改訂が進んでいる本邦の『膵癌取扱い規約』第5版(案)(最終校正第3回)では「膵管内腫瘍」を膵管内粘液性乳頭腫瘍((1)粘液産生型，(2)粘液非産生型)と膵管内管状腫瘍とに3分類している．粘液性嚢胞腫瘍の定義としてWHOやAFIP分類では卵巣類似間質が挙げられているが，本邦ではそれらは所見の記載に留まっている．その他，欧米と本邦の分類では異型度分類，浸潤癌の取り扱いでも異なっている．両疾患の診断と問題点につきreviewしてみる．

[ Summary ] Since the first report of "Mucin hypersecreting carcinoma of the pancreas with favorable prognosis" by Ohhashi et al., mucin hypersecreting tumors of the pancreas have drawn the attention of clinicians. In 1996, the WHO and the AFIP proposed new classifications for pancreatic tumors. Recently, the general rules for study of pancreatic cancer of the Japan Pancreas Society have been under revision. Recent controversies on the classification in western countries and Japan concerning the "mucin hypersecreting tumors of the pancreas" are reviewed.
