【 要旨 】 膵管内乳頭腫瘍(IPMT)の診断における経口膵管鏡(POPS),膵管腔内超音波(IDUS)検査の有用性と意義を検討した.切除されたIPMT78例に対し,POPSを全例に,IDUSを54例に施行し,病理組織診断と対比した.その結果,POPSにて血管透見を有するイクラ状隆起,絨毛状隆起,結節状隆起,およびIDUSにて隆起高5mm以上は上皮内癌以上の悪性を示唆する所見であり,その診断能は正診率がPOPS71%,IDUS67%で,両者を併用すると正診率は78%と向上した.これらの所見は切除手術の絶対適応と考えられた.また,POPSによる腫瘍の主膵管方向の進展度診断は80%以上の症例で可能であり,POPS,IDUSによる術前診断に基づいて切除範囲の決定を行ったところ,20%の症例で縮小手術が行われ,全体の3年生存率は95%,3年間無再発率は93%と良好であった.IPMTの鑑別診断,治療方針決定にPOPS,IDUSはきわめて有用と考えられる.

[ Summary ] Recently, peroral pancreatoscopy (POPS) and intraductal ultrasonography (IDUS) have been used as new diagnostic procedures for pancreatic diseases. In this manuscript, we intend to elucidate some useful findings concerning POPS and IDUS for the differential diagnosis of malignant and benign intraductal papillary mucinous tumors of the pancreas (IPMT), and to confirm the significance of these procedures in preoperative examinations. We have performed POPS on 78 and IDUS on 54 with histopathologically confirmed IPMT. Protruding lesions were identified in 55% by using POPS and 71% by using IDUS. Images of fish-egg-like with vascular objects, villous type objects, and vegetative type structures were seen by using POPS. Lesions protruding 5mm or more as seen with IDUS were considered to be malignant. Combined use of POPS and IDUS improved diagnostic accuracy. Tumor progression was successfully evaluated with POPS in about 80% of the cases. Partial resection was performed in 15 (19%) based on the results of POPS and IDUS. The 3-year cumulative survival rate and disease-free survival rate were extremely high at 95% and 93%, respectively. Although further improvement in the design of POPS and IDUS is required, they can be used to differentiate malignant tumors from benign IPMT and may provide useful information for determining the type of surgery to be performed.
