【 要旨 】 膵嚢胞性疾患を腫瘍性と非腫瘍性に分けて，その画像所見と膵嚢胞の診断の進め方について述べた．非腫瘍性嚢胞は画像所見のほかに膵炎や外傷などの既往，臨床経過および血清アミラーゼ値などの検査所見を参考に診断する．腫瘍性嚢胞は各疾患に特徴的な画像所見を描出することが大切であるが，適切な検査を取捨選択し，なるべく患者の負担を少なく鑑別診断を進め，外科的切除か経過観察かの治療方針を決定しなければならない．

[ Summary ] We described a method for differential diagnosis of pancreatic cysts by means of imaging. As for the non-neoplastic cysts, the examination of pancreatic enzymes such as amylase and anamnesis of previous pancreatitis and pancreatic trauma are helpful in doing imaging diagnosis. It is important when performing imaging diagnosis of neoplastic cysts to observe the characteristics of each tumor disease. We must choose the correct type of examination from several imaging modalities for the patient's benefit. In cases of neoplastic cysts, we must decide on whether surgical treatment should be performed or not, according to the results of the imaging diagnosis.
