【 要旨 】 肝内腫瘍性病変の診断にはfluorodeoxyglucose(FDG)-PETが用いられている.転移性肝癌や胆管細胞癌は肝細胞癌や良性腫瘍に比べstandardized uptake value(SUV)比は高値で,その鑑別に有用である.肝細胞癌ではSUV比が低値で癌の検出だけでは有用でない症例も多く存在するが,SUV比が高値の症例では肝細胞癌の治療効果の判定や再発の評価に有用である.さらに,SUV比は肝細胞癌の悪性度と相関を示し,肝細胞癌患者の予後を反映する.FDG-PETは肝細胞癌の診断において他の画像診断では得られない有用な情報を提供することができ,肝細胞癌の悪性度の評価や予後推定などに威力を発揮するものと期待される.

[ Summary ] Fluorodeoxyglucose(FDG)-PET is used for diagnosis of hepatic tumors. In a metastatic hepatic tumor or cholangiocarcinoma, the standardized uptake value(SUV) ratio is higher than in hepatocellular carcinoma or a benign hepatic tumors. This is useful for differentiation of the diseases. Although there are many cases of hepatocellular carcinoma in which the SUV ratio is low and is not useful for diagnosis, in cases with high SUV ratios, FDG-PET is useful for judgment of the effects of treatment of hepatocellular carcinoma or for diagnosis of recurrence. Further more, the SUV ratio is correlated with the degree of malignancy of hepatocellular carcinoma and is reflected in the prognosis for patients with hepatocellular carcinoma. FDG-PET provides useful information in the diagnosis of hepatocellular carcinoma that could not be obtained by other means of diagnostic imaging. Therefore, this examination is expected to be useful for evaluation of the degree of malignancy of hepatocellular carcinoma and formulation of its prognosis.
