| 【 要旨 】 | 硬度可変式内視鏡は，S状結腸をループ形成しながら挿入していく二人法に適したスコープである．S状結腸を通過するときには，スコープを軟らかい状態に設定し，押し込むように挿入していく．ループを解除してS状結腸を直線化した後は，スコープを硬くする．この硬度変換操作によって，S状結腸の再ループや横行結腸の大きな下垂が防止でき，深部大腸への挿入が容易になる． なお，スコープを硬くする操作は，必ず腸管が直線化された状態で行う．また，スコープに不自然な抵抗を感じたり，被検者が強い苦痛を訴える場合は，スコープを速やかに軟らかくして，偶発症の予防に努める． |

| [ Summary ] | A variable stiffness colonoscope is suitable for the two-men method, in which the scope is inserted with looping at the sigmoid colon. At the sigmoid colon, the scope was set in the soft mode and inserted by using the pushing technique. After undoing the loop and straightening the sigmoid colon, the scope should be changed to the hard mode. Changing scope-stiffness makes it easy to insert the scope into the proximal colon by preventing the sigmoid colon from re-looping and the transverse colon from hanging down. Changing scope-stiffness must be done when the sigmoid colon is straightened. If you feel any unusual resistance with the scope or the patient complains of strong abdominal pain, the scope must be softened quickly to avoid complications. |
