| 【 要旨 】 | H.pylori感染粘膜にはH.pylori陰性の正常粘膜に認められない所見 --胃底腺粘膜のびまん性発赤，点状発赤，胃体部ひだの腫大-- が内視鏡的に認められる．これらの3所見をH.pylori感染診断の指標とする通常内視鏡の総合診断では，感度94％，特異度81％であった．通常内視鏡観察後に行ったHemoglobin index(IHb)測定法およびコントラスト法ではそれぞれ感度91％，83％，特異度95％，95％と，いずれも良好な成績が得られた． 通常内視鏡の確診例以外の例に，IHbとコントラスト法を併用すると，診断成績は多少の向上にとどまったが，各検査法の利点が生かされ診断の確信性は増した． |

| [ Summary ] | Gastric mucosa affected with H.pylori (Hp) infection have a different appearance than Hp negative, normal mucosa, having diffuse redness, spotty redness in the fundic mucosa and swollen folds in the corpus. The results of endoscopic diagnosis of Hp infection, based on these 3 findings, were a sensitivity of 94% and a specificity of 81%. The same results, obtained by the application of Hemoglobin index (IHb) measurement, for which the cut off index was determined to be 60, were a sensitivity of 91% and a specificity of 95%. The results obtained by using the contrast method were 83% and 95% respectively. Subjects diagnosed as being suspect or negative for Hp infection, from above mentioned 3 characteristic findings of conventional endosocopy were rediagnosed, using results obtained with IHb measurement and the contrast method. The results for each diagnostic group rose slightly, but the substitution of other diagnostic methods created a more sure diagnosis. |
