【 要旨 】 Helicobacter pylori(H.pylori)未感染例の内視鏡像とともに,未感染胃に併存する胃病変について言及し,正常胃粘膜の考え方について述べた.またH.pylori感染によって生じる胃粘膜の特徴的な内視鏡像として,胃底腺粘膜領域のびまん性発赤,点状発赤,胃体部のひだの腫大,胃底腺,幽門腺領域の胃小区型の変動,腺境界型の上昇が認められることを述べた.
これらのH.pylori感染に特徴的な内視鏡所見を踏まえて総合的に判定していけば,H.pylori感染の有無を内視鏡像から高い確率で診断することができ,除菌診断にも応用が可能である.

[ Summary ] When examining the stomachs of individuals who were uninfected and those who were infected by Helicobacter pylori (H.pylori), we described the normal endoscopic findings of those uninfected with H.pylori, endoscopic findings of gastric lesions uninfected with H.pylori and endoscopic findings of those infected with H.pylori. The characteristic endoscopic findingsassociated with H.pylori infected gastric mucosa included : diffused, spotty redness in the mucosa of the fundic gland ; swelling of gastric folds in the body of the stomach ; changes in gastric areas of the fundic gland and pyloric gland ; and an advanced fundic pyloric border.
Based on these characteristic endoscopic findings of H.pylori infection, the presence or absence of an H.pylori infection can be diagnosed with a high degree of accuracy. This type of assessment is applicable in evaluating the eradication of H.pylori.
