【 要旨 】 | 超音波内視鏡検査(EUS)は7~20MHzという周波数の超音波を用い,局所の高い診断能をもつ検査法である.反面,減衰の影響を受けやすく,至適観察域はプローブから3cm程度までで,このような特性をわきまえて適応を選択する必要がある. EUSでは膵実質の変化,膵管の変化を描出することができる.前者については限局性,びまん性のechogenicityの変化をとらえることにより,画像診断の情報を提供する.後者では膵管造影法と同様,膵管径の変化を描出することにより,実質の変化と総合して病態を診断することを可能にする. 手技の習熟に時間を要するが,得られた画像情報は診断的価値が高く,今後,いっそうの普及が望まれる. |

[ Summary ] | Nearly 20 years have passed since the development of endoscopic ultrasonography (EUS). Because of its high spatial resolution, EUS is now widely used in diagnosing gastrointestinal diseases. With EUS, it is possible to depict local or diffuse changes in echogenicity of the pancreatic parenchyma, as well as to visualize changes in the main pancreatic duct and branch ducts. Although practice is required to master EUS procedures, images obtained by using EUS have high diagnostic value. More widespread use of EUS will contribute to early detection and precise diagnosis of pancreatic diseases. |
