【 要旨 】 レーザー内視鏡は約20年前に,内視鏡治療の先兵として大きな期待を担って登場した.内視鏡治療の分野では,レーザーが先導的な役割を果たし,その発展に大きく貢献してきた.しかし,高出力レーザーの熱エネルギーを利用した治療は,高周波やマイクロ波などを利用した治療でも可能になり,レーザーの応用範囲は狭められてきている.そして,現在は次世代のレーザーの応用として,その単色性を利用した低出力レーザーによる診断,治療に主力が注がれている.事実,新しい腫瘍親和性の光感受性物質とともに,長波長の半導体レーザーも開発されてきており,この分野において今後に大きな発展が期待されている.

[ Summary ] Twenty years ago, laser endoscopy appeared as the first endoscopic treatment and has contributed to the development of this type of treatment, However, this treatment which used heat energy of high power laser promoted to use of other methods, such as electocoagulation,or microwave coagulation. And the application of laser for endoscopic treatment has been gradually limited to specific cases, in which there are difficulties in performing operations and for endoscopic mucosectomy. At present, photodynamic therapy and photodynamic diagnosis of cancers are mainly being studied as the medical application of laser in the second generation,using low power laser systems and a fluorescence dye which can be absorbed in a targeted tumor tissue. And many new photosensitizers are capable of accumulating in malignant tumors more selectively, and new diode laser systems, releasing beams of longer wave lengths, which can reach deeper tissue, are being developed.
