Laparoscopic surgery for early gastric cancer is used for cases in which endoscopic mucosal resection (EMR) is not indicated because of tumor size or location. We present a method of laparoscopic intragastric mucosal resection, assisted by endoscopy. For safe and accurate laparoscopic intragastric mucosal resection, firstly, intragastric trocars must be inserted into a suitable site in the gastric wall to obtain a good visual field. Secondly, a sufficient quantity of saline should be injected into the submucosa to raise the mucosal lesion, avoiding intraoperative gastric perforation. Thirdly, a precise endoscopic marking of the clip preoperatively is necessary to designate the appropriate resection line and avoid any residual cancer. |
