【 要旨 】 現在，一般的に胃IIc病変に対する内視鏡的粘膜切除術(EMR)の適応は，深達度m，分化型腺癌，病変の大きさ1cm以下(または2cm以下)，潰瘍所見のないこととなっている．しかし，われわれの検討では，深達度m，分化型(高分化型，中分化型，乳頭腺癌)で潰瘍所見がなければ，病変の大きさまたは肉眼型に関係なく，リンパ節転移の可能性はきわめて低く，EMRの適応と考えられたまた，同時性・異時性の多発癌でも，各病変が同様の条件を満たせば，それらもEMRの適応と考えられた．

[ Summary ] From the study of the specimen of early gastric cancer operated on at our institute, the rate of lymph node metastasis of all differentiated mucosal gastric cancers without ulcerative changes was extremely small, regardless of individual maximum diameters or macroscopic type. Therefore, it is suggested that all of them may be treated radically by using endoscopic mucosal resection.
