| 【 要旨 】 | 食道アカラシアは，下部食道の機能異常に由来する良性疾患である．一般に治療の第一選択としては拡張術を主体にした保存的治療が行われ，改善がみられないときに手術に踏み切る場合が多い．外科的治療の目標は，通過障害を改善し，逆流を防止することである．歴史的に，この要求に答えるべくさまざまな術式が開発されてきた．元来良性疾患であることから，低侵襲手術が好ましく，近年，内視鏡的手術の報告もみられる． 本稿では当科における手術適応，現在広く行われている手術方法および術後合併症について概説した． |

| [ Summary ] | Achalasia is a functional disease characterized by failure of lower esophageal sphincter relaxation. Medication given initially is usually unsatisfactory. Then, in patients who do not respond to such therapy, surgical treatment is indicated when pneumatic dilatation does not improve the symptoms. To relieve functional obstruction and prevent gastro-esophageal reflux, various surgical procedures have been developed. Recently, endoscopic surgery has been applied to the treatment of achalasia. We review the surgical treatment and procedures for managing achalasia from the historical aspect. We also discuss the indications for and complication of these surgical procedures. |
