【 要旨 】 わが国のPSC症例192例の特徴として,欧米の症例に比し平均年齢が高く(47歳),炎症性腸疾患の合併率が低く(21%),慢性膵炎の合併率が高い(15%)という特徴がみられた.また,年齢分布は2峰性を示し,40歳未満の症例で,炎症性腸疾患の合併頻度が高く(39%),慢性膵炎の合併頻度が低く(4%),罹患胆管でも肝内と肝外胆管の両方にみられる頻度が高かった(89%).このことは,40歳以上の症例のなかには異なった病因の症例が含まれていることを示唆しており,PSCの診断に関してはこの点に留意するとともに,今後,診断基準を含めた検討が必要と考える.

[ Summary ] Characteristics of primary sclerosing cholangitis (PSC)cases in Japan include a higher average age (47 yrs), a lower incidence of complications of inflammatory bowel diseases (21%) and a higher incidence of complications of chronic pancreatitis (15%), in comparison to American and European cases. Furthermore, two peaks were observed in the age distribution of PSC cases in Japan. Patients younger than 40yrs had a higher incidence of complications of inflammatory bowel diseases (39%), a lower incidence of complications of chronic pancreatitis (4%) and a higher incidence of damage involving both intra- and extrahepatic bile ducts (89%).
These findings indicate that cases of different etiologies are included among PSC patients older than 40 yrs in Japan. In the diagnosis of PSC, we need to consider these points, and further studies including criteria for diagnosing PSC are required.
