【 要旨 】 クローン病自体の治療法の基本が最近20年間あまり変化がないのに対して，その合併症の治療法は，従来有効なものが少なかったこともあり変化を余儀なくされている．しかし，内科的治療，外科的治療とも完全に満足できる合併症の治療法は原則的には存在しないと現在の時点では考えるべきである．そのため，現状のクローン病の合併症の治療において重要な点は，「内科的に栄養状態，炎症をコントロールした後に，minimalな腸管切除術を行う治療」にみられるように，内科的保存療法と外科的治療法のcombinationをうまく行い，QOLを含めた患者の予後をできるだけ長期間良好に維持することである．

[ Summary ] Even though there has been significant progress in the treatment of Crohn's disease patients with complications, no single intervention is sufficient for treating an individual complication. Therefore, combining medical and surgical management should be considered as a means of controlling these complications and maintaining patients' QOL.
