【 要旨 】 クローン病は潰瘍性大腸炎と異なり，完全静脈栄養法(TPN)あるいは成分栄養による経腸栄養療法が，単に低栄養状態を改善するのみならず，腸管の炎症そのものを改善する．本項ではクローン病に対する完全静脈栄養法(TPN)の適応，TPNの施行法の実際，在宅TPN,TPNで投与する栄養素，脂肪乳剤，グルタミン補充，微量元素，TPNの合併症とその対策などについて解説する．

[ Summary ] Nutritional therapy with total parenteral nutrition (TPN) or an elemental diet not only improves malnutrition in Crohn's disease (CD) but effectively inhibits the inflammatory process of Crohn's disease. This feature distinguishes CD from ulcerative colitis. This article reviews indications for TPN in CD, how to start and maintain TPN, home-TPN, nutrients administered in TPN, lipid emulsion, supplementation of glutamine and trace elements, various complications of TPN and their management.
