| 【 要旨 】 | NUDの定義は「器質的疾患の除外」と「上腹部不定愁訴の慢性的持続」の2点に重点が置かれている．NUDはその症状により(1)gastroesophageal reflux-like dyspepsia，(2)dysmotility-like dyspepsia，(3)ulcer-like dyspepsia，(4)non-specific dyspepsiaの四つに分類されている．治療法はそれぞれの型により異なり，消化管運動改善剤・粘膜保護剤・制酸剤・酸分泌抑制剤・向精神薬が用いられる．これら西洋薬で改善傾向を示さないNUD患者に対しては漢方薬による治療が試みられるが，これらの方剤のなかでは六君子湯がもっとも頻用されている．NUDの診断に際しては慎重な病歴の聴取に始まる西洋医学的な検査が必要であり，臨床症状のみから安易にNUDと診断して重篤な他の疾患を見逃すことがあってはならない． | |

| [ Summary ] | Non-ulcer dyspepsia (NUD) has very common clinical symptoms, such as abdominal distension, bloating, nausea, and post-prandial discomfort. Disturbed gastrointestinal motility is common in these patients, and appears to play a major role in the development of NUD. Despite the many drugs (e.g.prokinetic agents, H2 blockers) introduced, NUD is often resistant to treatments because of its heterogeneity (reflux-like, dysmotility-like, ulcer-like and non-specific dyspepsia). Among many traditional Chinese herbal recipes, some are indicated to be effective for NUD. The traditional Chinese herbal medicine "Rikkunsi-to" has a prokinetic action on gastrointestinal motility, and has clear clinical effects on NUD. | |
