【 要旨 】 透析者はワクチンによる抗体獲得率が健腎者より低く,ツベルクリン反応が陰転化し結核の罹患率が高くなるといった液性・細胞性の両方の免疫能の低下が認められる.統計調査の死因分類の第2位が感染症で,肺炎が約半分・敗血症が4割であった.さらに肺炎の4割,敗血症の3/4がメチシリン耐性を含めた黄色ブドウ球菌による感染症であった.感染症起因菌の偏りから生命予後にかかわる生体防御の障害の主体は,免疫細胞による菌微生物の貪食機能の低下と皮膚・粘膜防御の破綻と考えられる.しかしこの障害は管理可能であり,透析者の個人衛生(サニテーション)と,MRSAなど耐性菌を積極的に探して保菌のうちに駆除する方法で感染症を減らせると考えられる.

[ Summary ] Patients undergoing hemodialysis show lower levels of both, humoral and cellular immunity; thus, the antibody acquisition rate following vaccination is lower than that observed in healthy individuals with normal renal function. These patients show a negative tuberculin reaction with an increase in the prevalence rate of tuberculosis. A statistical survey demonstrated that infectious diseases accounted for the second most common cause of death-pneumonia accounting for approximately 50 % and septicemia for 40 % of the cases. Furthermore, 40 % of pneumonia and 75 % of sepsis cases were attributable to infections caused by Staphylococcus aureus including methicillin-resistant organisms. The most important disorder of biological defense related to vital prognosis from bias of infectious disease causative bacteria is thought to be a decline in the phagocytic function of microorganisms and breakdown of skin defense. Adequate sanitization measures need to be adopted in those undergoing hemodialysis in addition to methods to eradicate methicillin-resistant Staphylococcus aureus colonization.
