【 要旨 】 慢性腎臓病における骨折発生率は高い.さらに,透析を受ける末期腎臓病患者に生じた骨折は高い死亡率を示すことも知られている.副甲状腺機能亢進症・骨軟化症・無形成骨などさまざまな骨病態を有する透析患者に骨折は生じる.現在のところ,正常腎機能を有する男女における骨粗鬆症性骨折予防の大黒柱はビスフォスフォネートである.しかし,透析患者に対するビスフォスフォネートの安全性・有効性を前向きに調べた研究はない.一般的に,小児においては運動負荷を与えると骨強度は増大するが,成人には当てはまらない.ましてや透析患者は疲労感のために著明な活動制限を示す.身体活動性改善を目的とした貧血の治療も含んだリハビリテーション戦略が望まれる.

[ Summary ] Susceptibility to fractures is increased across the spectrum of chronic kidney disease patients. Moreover, fractures in patients with end-stage kidney disease result in significantly increased mortality rates. Fractures develop in hemodialysis patients with hyperparathyroidism, osteomalacia, and adynamic bone disease. To date, bisphosphonates are the mainstay for prevention of osteoporotic fractures in men and women with normal kidney function. However, there are no prospective studies which address the safety or efficacy of bisphosphonates in hemodialysis patients. In general, exercise can significantly enhance bone strength at loaded sites in children but not in adults. In addition, patients receiving hemodialysis report a significant degree of immobility due to fatigue. Clinical application of rehabilitative strategies, including therapies for anemia to improve physical functions in patients on hemodialysis, is greatly needed.
