【 要旨 】 ガイドライン出版直前や出版後に上市された新薬は，ガイドラインに掲載されることは少ない．本稿では，シナカルセト塩酸塩を例にとり，ガイドラインに掲載されない新薬が実臨床でどのような薬効を期待され，どのように処方されているのか，上市後どのようにエビデンスが形成され，次期のガイドラインに掲載されるようになるか，使われなくなるかを論じた．本考察が，臨床医の新薬処方に対する取り組みの一助になることを期待する．

[ Summary ] New drugs which are approved near the time of guideline publication or after guideline publication seldom appear those guidelines. Taking cinacalcet as an example, we discussed how a new drug's clinical effects were anticipated in clinical medicine, how they were prescribed, and how the evidence was formulated after approval. We hope this discussion will help clinicians to understand how new drugs emerge in revised guidelines.
