| 【 要旨 】 | 透析患者の精神・心理的側面を考え，支えていく場合の透析医の日常臨床での心がけを記した．精神・心理的な問題を考える場合，常に身体医として，原疾患や脳，代謝，透析不足，薬剤の (副) 作用など広く器質的な疾患との関連性を考慮する必要がある．そのうえで，透析患者に特徴的な問題なのか，高齢に伴う病態，糖尿病性腎症の有無などを考え，精神，心理的側面の出ている問題の中心的役割を果たしている病因を，身体的，脳器質的，心理的などに分類してみる．また，軽い意識障害を見逃さず，せん妄，うつ，不眠，精神科疾患などを鑑別にあげる．こうしたアプローチのうえで，精神科医に十分な情報を提供し，患者が参加した形でコンサルテーションし，精神・心理的問題の解決に向けコラボレーションしていくことが必要である． | |

| [ Summary ] | Our attitude toward hemodialysis patients with psychological problems is described. When considering psychological problems, underlying conditions such as brain disorders, metabolic disorders, insufficient hemodialysis therapy and drug side effects, must be kept in mind. Hemodialysis patients' problems need to be classified into physical, brain or mental categories. We also must consider whether hemodialysis patients have diabetic nephropathy or conditions associated with old age. Mild disturbances in consciousness should be dismissed. However, delirium, depression, insomnia, and other psychiatric disorders must be paid attention to. Lastly, we must collaborate not only with psychiatrists but also hemodialysis patients in order to treat their psychological problems. |
