【 要旨 】 介護保険法改定による予防重視型システムへの転換に伴い，軽度者のサービスの見直しが始まる．これにより要介護1の大部分が要支援へ移行し，新しく創設された予防給付による介護予防サービスが提供されることになる．外来透析患者では介護度が比較的軽度の場合が多く，要介護1の占める割合も高い．このため何割かが要支援に移行すると考えられる．しかし，予防給付となった場合は外来透析患者がもっとも必要とする通院乗降介助サービスは利用できず，選択できるサービスも限られたものとなる．中・重度となった場合の介護施設入所困難も未だに解決されず，今後の外来透析患者の在宅生活が危ぶまれる．

[ Summary ] Reassessment of handicapped individuals receiving low levels of care was perfomed, in relation to revisions of the Japanese Long Term Care Insurance system. There are many patients receiving low levels of care in outpatient dialysis facilities. Their disabilities must be reevaluated in relation to lower degrees of care provided. The most indispensable service for these patients is assistance in getting to the hospital three times a week, which is not available if they are re-classified as having a lower degree of disability. This revision will be especially disadvantageous to dialysis patients.
