【 要旨 】 阪神・淡路大震災は初めて人口密集地域を襲った大規模な透析施設の被災として，透析医療に携わるものにとって貴重な教訓となった．2004年10月23日，震度7の地震が新潟県中越地方を襲ったときこの教訓は活かされたであろうか．透析室の被害状況は3病院336名が治療の場を失い，バスなどで他施設へ搬送治療を行い，透析施設の機能は約1週間で復旧した．この直下型地震で被害が最小限に抑えられたのは，地域の透析医の連携で患者移動がスムースにいったことがあげられる．この教訓から都区部での直下型地震に対する災害対策を考えるとき，災害により透析不能となった患者をどこで透析をするのかという，代替施設確保がもっとも中心に据えられる．これに対応するために，2005年，東京都区部災害時透析医療ネットワークが立ち上げられた．この動きを紹介し，人口密集地における直下型地震対策について述べる．

[ Summary ] The Kobe earthquake caused suffering great huge numbers of chronic dialysis patients by depriving them of dialysis therapy. The Chuetsu earthquake also deprived dialysis therapy from 336 patients in three hospitals. Those patients were transferred by bus to other facilities for a week. In planning counter measures for dialysis patients in the event of an earthquake with it's epicenter in Tokyo, establishment of alternative facilities is vital. We came up with the idea of a network for dialysis patients in natural disasters. We described the framework for the “Tokyo Network for Dialysis Patients in Natural Disasters”.
