| 【 要旨 】 | 生命維持のために慢性透析の継続を必須としている患者が，種々の要因から透析中止に陥る状況は皆無ではなく，近年，むしろ増加している傾向にある．透析の中止には，(1) 患者に発生した重篤な合併症が，透析という体外循環や腹腔内洗浄の処置を，主として身体的な理由から不可能にする事態と，(2) なんとか透析操作が可能であっても，患者自身がこれを拒絶する事態，さらに，(3) 患者自身には意識がないか判断力に欠けるが，善意の第三者が中止を選択する場合，などの状況が存在する．いずれの状況下であっても，最終決定に際し医療側・家族側を苦慮せしめるものである．事前指示書が未だ習慣となっていない日本では問題点も多く，いくつかの側面から考察した． | |

| [ Summary ] | In recent years, the termination of dialysis for various reasons in patients requiring chronic dialysis for life support has been on the increase. Among the reasons for termination are: 1) Serious complications that make it physically impossible to perform extracorporeal circulation or intraperitoneal lavage as needed in dialysis treatment. 2) Dialysis is performable yet the patient refuses the treatment. 3) The patient is unaware of the situation, lacks the power of judgement or a third party acting in good faith decides on the termination of dialysis. Whatever the situation, when it comes time to make the final decision those in the medical community as well as the family are extremely troubled. There are many problems in Japan where advance directives are not yet the norm, a number of facets are considered here. |
