Theme Endscopic Ultrasonography for Colorectal Cancer
Title Imaging of the colorectal cancer using 3D-EUS (three dimensional endoscopic ultrasonography) system

[ Summary ] Diagnostic abilities of colorectal cancer were observed using 3D-EUS (three dimensional endoscopic ultrasonography) system.
The ultrasound 3D imaging system (ELT-IP-I, EU-IP-II) developed by Olympus Co. was composed of an ultrasound 3D imaging processing unit and an ultrasound probe. The ultrasound probe inserted in the sheath was spirally moved and imaged radial images (40-120 slices) and also imaged linear one simultaneously. 3D images (oblique projection image ; OPI, surface rendering image ; SRI) was composed after examination by computer processing. This system also assemble other function of high resolution mode and volume measurement etc. in case of necessity.
After same preparation as colonoscopy, 3D-EUS was performed with water filling method. In the case of mucosal cancer, most of cases were easily visualized only radial image, but lesions on the fold sometimes disturbed radial image and it solved linear image of 3D-EUS method. 3D images sometimes reflected real shape of lesions as virtual endoscopy, but sometimes failed to visualize faithfully because of the difficult shape.
We experienced some cases of cancer of submucosal invasion, which were provided more precise image under linear image than the ordinary radial image. Advanced cancer also imaged clearly using 3D image to detect the swelling of metastatic lymph nodes. Functions of high resolution image was useful to observe the layer structure more precisely and volume calculation may be useful to examine the size of submucosal tumor.
These 3D image helped to understand the lesions more spetially as virtual endoscopy. Problems (echo dumping, pulsation) still exist but there must be solution according to the advantage of technology.
