【 要旨 】 大腸SM癌のSM浸潤度は,SM層での癌浸潤距離が1,000μmを基準線にSM軽度浸潤癌とSM高度浸潤癌に分類する.SM浸潤度は,SM癌の転移・再発の危険性を術前に予測できる指標であり,適切な治療法を選択するのに役立つ.SM浸潤度の術前診断には通常内視鏡観察を基本として用いるが,診断が難しい場合は超音波内視鏡や拡大内視鏡によるpit patternの評価やNBIなどの画像強調観察を併用する.しかし,これらの精密検査法を用いても,その診断精度には限界があることを理解しておく必要がある.

[ Summary ] Colorectal submucosal cancer can be divided into two groups according to whether the depth of submucosal invasion is less than 1,000 μm or 1,000 μm or greater. The first is submucosal cancer with slight invasion and the second is submucosal cancer with massive invasion. The depth of submucosal invasion can be used as an index to preoperatively predict the risk of metastasis and recurrence in patients with submucosal tumors and is useful for selecting appropriate treatment strategies. Conventional endoscopy is primarily used to diagnose the depth of submucosal invasion. If it is difficult to make a diagnosis, endoscopic ultrasonography, evaluation of pit patterns through magnifying endoscopy, and image-enhancement techniques such as narrow-band imaging should be used. However, it is important to understand that the diagnostic accuracy of these techniques for determining-depth is limited.
