【 要旨 】 大腸の粘膜下層は場所によって厚さが大きく異なること，また，内視鏡的摘除標本は粘膜下層のどこで切離されたのか不明なうえに固有筋層がないのでSM浸潤度相対分類は使用不能である．そこで，大腸癌研究会の「大腸SM癌取扱い」プロジェクト研究の結果，SM浸潤度の評価法として浸潤距離実測法が提示され，2005年版の「大腸癌治療ガイドライン」に盛り込まれた．結果として，超微小SM浸潤癌のみでなく，他の病理学的条件（組織型，脈管侵襲など）を満たせば，SM浸潤1,000μm程度まで根治と判定できるようになった．
