【 要旨 】 タクロリムス（tacrolimus）は現在，本邦で難治性潰瘍性大腸炎に対して使用されている．ステロイド抵抗性，依存性の潰瘍性大腸炎患者に有用であり，その使用頻度は高まってきている．タクロリムスはトラフ依存性に濃度有効性を示す薬剤であり，十分な効果を得るために投与上の工夫が必要である．また，従来用いられてきたシクロスポリンや最近，潰瘍性大腸炎に対して承認されたインフリキシマブとの位置づけについては未だ明らかなコンセンサスはない． 本稿では具体例を示しつつこれらの課題について概説する．

[ Summary ] Tacrolimus is currently used in Japan for intractable ulcerative colitis treatment. This treatment is useful for patients with UC who exhibit resistance to or dependence on steroid therapy. The frequency of tacrolimus use for UC treatment has increased recently. It is well─known that the effects of tacrolimus depended on the trough level. There is no evidence of recommended treatment strategies or indications for use of tacrolimus. We outline these problems and demonstrate cases of UC treated with tacrolimus.
