[ Summary ] The authors have invented short needle knives which can emit a jet of water from the tip of a sheath (FlushKnife™) in order to perform endoscopic submucosal dissection (ESD) more easily, safely, and efficiently. Shortening of the projecting section of the tip of needle knives has enabled us to perform small sharp marking, easy safe incisions of the mucosa, as well as submucosal dissection. Emitting a jet of water from the tip of a sheath provides lavage of the viewing field, identification of the bleeding points, and submucosal local injection without replacement of operative instruments or the knife itself. This has led to very efficient treatment. The Flush Knife™ can be considered to be one of the most useful operative instruments for ESD.
