[ Summary ] |
Ingestion of raw carp gallbladder is a rare cause of acute renal failure. The patient initially presented gastrointestinal symptoms including nausea, vomiting, diarrhea, and abdominal pain. These symptoms occurred within a few hours after ingestion. Many cases developed acute renal failure and toxic hepatitis. The diagnosis is primarily clinical, and no specific treatment has been identified; renal and hepatic impairment generally resolve with supportive care within a few weeks after the incident. A previous study reported that 5 alpha-cyprinol sulphate, a component of carp bile, may be responsible for the pathogenesis of this syndrome. It is thought that 5alpha-cyprinol sulphate may have a toxic effect directly on the kidney and liver. Herein we reported on a case of oligouric acute renal failure following raw carp gallbladder ingestion, in which temporary hemodialysis treatment was successful. Renal biopsyrevealed acute tubular necrosis. A review of 25 Japanese cases reported previously in literature is also described. |
