【 要旨 】 | わが国の原発性骨粗鬆症診断基準の最新版である「原発性骨粗鬆症の診断基準(2012年度改訂版)」および薬物治療開始基準を示した最新版である「骨粗鬆症の予防と治療ガイドライン2011年版」はわが国の骨粗鬆症診療の根幹をなすものである. 前者の診断基準は2000年度改訂版における問題点を整理し,国際的な整合性をはかるとともに,この12年間に蓄積された新たなエビデンスを織り込んでいる.一方,後者のガイドライン2006年版では初めて薬物治療開始基準が示され,骨折リスク指標をも加味した.この基準から4年が経過した2011年版では,各種既存骨折種のうち大腿骨近位部骨折と椎体骨折を重視するとともに,FRAX®のカットオフ値を具体的に示した.このことは,わが国では大腿骨近位部骨折が未だに減少していないので,積極的な治療介入を期待するものである. |

[ Summary ] | The Revised 2012 Diagnostic Criteria for Primary Osteoporosis and the Criteria for Initiation of Pharmacological Treatment as part of the 2011 Guidelines for Prevention and Treatment of Osteoporosis have been combined to provide the cornerstone for the management of osteoporosis in Japan. The former addresses several issues that remained unresolved in the earlier 2000 criteria, ensures consistency with global standards of care, and reflects the new evidence accumulated during the intervening 12 years. On the other hand, the latter represents an updated version of the 2006 guidelines, which provided, for the first time, criteria for initiating pharmacological treatment in osteoporosis based on fracture risk. However, this time, the focus is mainly on proximal femoral fractures and vertebral fractures. Moreover, specific FRAX® cut-off values for the indication for osteoporosis screening are presented, which is of particular interest in encouraging aggressive therapeutic interventions in patients at high risk of osteoporotic fractures, particularly in those at risk of proximal femoral fractures, which still remain less well controlled in Japan. |
