| 【 要旨 】 | 透析医療における意思決定は，従来，個別の治療選択に限定されてきたが，患者の高齢化と症状負担の増大に伴い，価値観に基づく継続的プロセスとして再構築する必要がある．本稿では，shared decision making（SDM）とadvance care planning（ACP）の概念と変遷を整理し，両者を統合した意思決定枠組みを提示した．ACPは終末期に限定されず透析導入早期から開始し，病状変化に応じて反復的に実施すべきであり，対象はすべての透析患者に及ぶ．SDMとACPは，価値に沿った医療を実現する基盤的プロセスである． | |
