【 要旨 】 全人的医療(biopsychological medicine)とは患者の視点をもつことを強調した医療である.高齢化した透析患者のADLは低下しており,介護度が高い患者が多くなってきている.したがって,患者の介護度に合わせた療養環境を整える必要があり,さらに維持血液透析のための通院手段に配慮する必要がある.長崎県での実態調査では,透析患者の認知症の頻度は高く,服薬アドヒアランスは悪く,摂食障害がみられ,誤嚥性肺炎などが生命予後に関わっていた.認知症は早期に発見し,認知症外来を受診して日常生活の継続をはかる.ADLの低下および認知力の低下した透析患者に対しては,患者および家族を中心に種々の職種が関わって患者を支えていく必要がある.

[ Summary ] Holistic medicine (biopsychological medicine) emphasizes the person as a complete being and the combination of human mind, body, and spirit. Owing to decreased levels of activities of daily living (ADL) in elderly dialysis patients and an increased number of patients who require high-level care, a suitable medical environment with the required level of care, is needed. In addition, the reasons for which patients visit the hospital for maintenance hemodialysis needs to be considered. In dialysis patients, the frequency of cognitive impairment is high and the compliance to drug treatment is poor. Eating disorders and aspiration pneumonitis in patients undergoing hemodialysis can compromise the prognosis and survival. However, if the cognitive impairment is discovered early, dialysis patients can plan their visit to the outpatient dementia department and integrate their treatment care plans into their daily lives. Therefore, dialysis patients with decreased ADL and impaired cognitive abilities need quality care with a patient-centric focus from various professionals with different specialties.
