[ Summary ] The United States National Colonoscopy (NCS), funded by the National Cancer Institute of the Unites States, looking at screening colonoscopy versus usual care in different clinical settings in the United States, has successfully gathered a large body of useful data. However, techniques used by many international centers for the study of flat and depressed adenomas, such as dye spraying, magnification and pit pattern recognition were not utilized in the initial colonoscopy cohort. Thus an opportunity to look prospectively for flat adenomas in a multi institution US Screening colonoscopy trial may not have been optimum. There has been increasing interest in the United States, and among US colon cancer researchers, about the role of the flat and depressed adenoma.
The NCS study continuation, contrasting colonoscopy with fecal occult blood directed colonoscopy, gives the NCS a chance to look more closely at flat and depressed adenoma in our population.
