[ Summary ] Precise assessment of mucosal features is important for the endoscopic diagnosis of digestive tract diseases. We have developed a new endoscopic system: Narrow Band Imaging (NBI) system, which is based on technology using narrow bandwidth spectral transmittance through optical filters, used as a sequential lighting method for an electronic endoscopic system. The present study described the NBI system for in vivo diagnosis of neoplasms of the colorectum during routine clinical endoscopy.
Methods: In total, twenty seren lesions of the colorectum in twenty patients were included. Lesions were evaluated three times each, using an endoscopic system with conventional and NBI filters, and with chromoendoscopy. Endoscopic diagnoses were compared to histology.
Results: Seven hyperplastic polyps, nineteen adenomas and one early cancer were detected. The detectability of micro vessels in the neoplasm surface and mucosa with the NBI system was significant compared to that with a conventional lighting system (p<0.0001). The appreciative observation of pits with the NBI system was equivalent to that with chromoendoscopy.
The NBI system could differentiate lesions with accuracies of 93%, which was equal to that of chromoendoscopy.
Conclusion: This study suggested that the NBI system might be simple and useful for the preciseestimation of colorectal neoplasms, resulting in accurate quantitative and existential diagnosis.
