Theme Elucidation of the pathogenesis of rectal carcinoma with special reference to colonic carcinoma
Title Chronological changes of the clinico-pathological features of colorectal cancer in Japan

[ Summary ] Background : The trend has been toward a continuous increase in the number of colorectal cancer cases over the last 50 years. Overall treatment results for rectal cancer patients appear to have improved in Japan. On the other hand, the increased number of less advanced cancers has promoted the introduction of less invasive techniques with the intent of preserving better anal, urinary and sexual functions. An overview is presented of the chronological changes in the clinico-pathological features of the colorectal cancer in Japan over the last 20 years. These can be referred to for examinations of the significance of these newly introduced treatment procedures.
Materials : Reports (1-17) of the "Multi-Institutional Registry of Large Bowel Cancer in Japan" published by Japanese Society for Cancer of the Colon and Rectum, and reports from the research group of the regional cancer registry were alsoused.
Results : The relative incidence of the colon cancer to the rectal cancer increased two fold during the latest 20 years. Relative incidence of superficial type cancer and submucosal cancer of the rectum also increased 3.1 times and 3.0 times, respectively, in the same period. In 1996, sphincter saving procedures were used three times more often as the definitive treatment for rectal cancer than in 1974. Conversely abdomino-perineal procedures decreased from 74.9% to 28.6% during the same period.
