[ Summary ] Polypoid colorectal tumors, which grow mainly in a lateral direction, were first reported in 1979 by Satake et al. In addition of flat type to these tumors, the concept of “Laterally spreading tumors (LST)’ was introduced in 1993 by Kudo et al. The recognition and development of this concept was established through the use of magnifying endoscopy, which has brought the use of clinicopathological diagnosis to endoscopic diagnosis.
Oda et al, proposed the endoscopic definition of flat type LST as superficial tumors in which the edge of the tumors display a “Pseudopodic appearance’ and IIIL pit pattern, as seen with magnifying endoscopy.
Flat type LSTs were compared with depressed colorectal IIc tumors, because of their morphological or functional similarity. Both types of tumors showed negative K-ras mutation and positive diffuse p53 mutation, as differentiated from polypoid tumors. This was especially so with pseudo-depressed type of flat type LST, which drew attention to the similarity of function related to vertical invasion. The main controversy centered around whether vertical invasion is related to the potency of tumorgenesis or length of exposure.
In recent years in Japan, endoscopic submucosal dissection techniques have been developed. The integrity of LST research has helped in the treatment of large colorectal tumors, such as LST.
