【 要旨 】 クローン病において小腸病変評価は治療方針を決めるうえで重要である．バルーン内視鏡を用いることでクローン病の小腸粘膜病変を詳細に評価することが可能である．定量的な内視鏡評価は重要であるが，小腸をスコアリングする内視鏡指標は限定的であり，当施設での内視鏡評価方法および内視鏡スコアについて紹介する．またバルーン内視鏡は臨床的に非常に有用である一方，検査可能な術者や施設が限られる点などを考慮すると，内視鏡の代用となりうる検査は重要な課題である．クローン病の小腸病変に対するMRE検査やバイオマーカーについても記述する．今後，バルーン内視鏡の利点・欠点をよく検討し，その他の検査も含めた診療のストラテジーを構築する必要がある．
