【 要旨 】 Mirizzi症候群は慢性胆嚢炎および胆石症のまれな合併症で，胆嚢頸部や胆嚢管の結石による圧排，炎症性変化が近接する総胆管の一部または完全狭窄をきたす病態である．外科的治療が第一選択であるが，McSherry type Ⅱに対しては経口胆道鏡を用いた内視鏡治療の有用性も報告されている．術前診断が困難であることが特徴の一つであるが，病態の詳細な検討は治療方針決定に重要である．本稿ではMirizzi症候群の診断・治療につき概説する．
